LEADR program

About the LEADR Program


Lifestyle Empowerment Approach for Diabetes Remission (LEADR) is an evidence-informed program developed by the American College of Lifestyle Medicine. It is designed to help participants take control of their health through sustainable lifestyle changes that benefit both individuals and their families.


Participants will learn how to:


  • Eat for diabetes remission—without counting calories

  • Achieve a healthy weight without experiencing hunger

  • Improve energy levels and overall quality of life


Eligibility


LEADR is open to individuals diagnosed with:


  • Type 2 diabetes

  • Prediabetes

  • Nonalcoholic fatty liver disease (NAFLD)

  • Metabolic syndrome

  • Insulin resistance


Additional requirements:


  • Participants must be at least 16 years old to enroll as the primary patient

  • A primary support person is encouraged to participate


Please note:
This program is not intended for individuals with type 1 diabetes, an autoimmune condition that affects the pancreas.


Cost


As part of a pilot study, this program is evaluating insurance billing and reimbursement processes.


  • With insurance: Sessions will be billed to your plan. You will not be responsible for costs if claims are denied.

  • Without insurance: A sliding scale fee is available to ensure accessibility for all participants.


Pilot Program Information


LEADR is currently being offered as part of a multi-site pilot across the United States.

A pilot program allows for:


  • Testing in real-world clinical settings

  • Gathering feedback from patients and providers

  • Identifying and resolving implementation challenges

  • Evaluating program effectiveness and feasibility


Feedback collected during this phase will help refine the program and support broader implementation.


Program Sessions


LEADR offers flexible participation options:


  • Telehealth: Live sessions via Zoom

  • In-person: At our clinic


Program structure:


  • 13 weekly group sessions

  • Approximately 90 minutes per session


Scheduling:


  • Anticipated start date: June 2026

  • Telehealth sessions: Saturday afternoons

  • In-person sessions: Friday evenings


We encourage interested participants to share their preferred days and times. We will be offering this program on a continuous basis with future offerings based on patient requests.
