What is the LEADR program?
Lifestyle Empowerment Approach for Diabetes Remission (LEADR) was developed by the American College of Lifestyle Medicine. It is an evidence-informed program that helps patients take control of their health through sustainable lifestyle changes—benefiting both individuals and families. Participants learn to eat for diabetes remission without calorie counting, achieve a healthy weight without hunger, and improve energy and quality of life.
Who is eligible to participate?
Anyone who has been diagnosed with type 2 diabetes, prediabetes, nonalcoholic fatty liver disease (NAFLD), metabolic syndrome, or insulin resistance. Patients must be 16 or older to be enrolled as the primary patient. We encourage participation of your primary support person. Please note that this program is not designed for those with type 1 diabetes, which is an autoimmune disease that targets the pancreas and destroys its ability to function properly.
What is the cost?
One of the goals of the pilot study is to determine any difficulties with billing and receiving reimbursement from insurance companies for the program. If you have insurance, the sessions will be billed to them. If the claims are denied, you will not be responsible for the cost. If you do not have health insurance you are still able to participate. We have a sliding scale fee that will be applied.
What is a pilot site?
A pilot site is a location where a new program is first tested on a small, controlled scale to gather data and feedback before a full-scale implementation. It's a preliminary stage, like a dress rehearsal, used to identify potential issues, refine processes, and assess feasibility and effectiveness in a real-world environment..
The American College of Lifestyle Medicine is conducting a multi-site pilot, which means there are a number of sites that have been chosen across the country. They will be collecting feedback on patient and medical provider experiences, the ease of implementation, and the success in billing and reimbursement from insurance companies.
Where and when will sessions be held?
There will be options to participate either through telehealth on the Zoom platform or in person at the clinic. The program consists of 13 weekly group sessions that are approximately 90 minutes long. Exact dates and times have not been determined, and we are asking interested patients to let us know which days of the week and times would work best for them. The anticipated start date for the program is January 2026.
